Program Manager Community Care

We are seeking an experienced person on a full-time basis to manage the delivery of ComPacks packages across Goulburn-Mulwaree and Upper Lachlan shires from a Goulburn base.
The ComPacks program provides access to a short-term package of in-home care to facilitate safe and early discharge of eligible people from public hospitals to regain independence and prevent re-admission.
Interchange Australia is a progressive community care provider based in Bowral assisting people with disability, the frail-aged, carers and other eligible people to live independently in their community since 1986. Our services include home care, respite, leisure, case management, ComPacks, living and learning, dementia monitoring, ASD specific programs and accommodation. Interchange Australia has recently won a contract to deliver ComPacks and other packages in the Southern Local Health District.
Key responsibilities of the role include but are not limited to:
/gAssessing clients in hospital and at home when
required
/gDeveloping person centered care plans
/gCo-ordinating, monitoring and reviewing in home
services
/gLiaising with hospital staff, family members,
carers and other stakeholders
/gMaintaining accurate and up-to-date records
Essential requirements for this role:
/gDemonstrated experience in co-ordination,
monitoring and reviewing of in-home care
services for people with disability and people
who are frail-aged
/gRelevant tertiary qualification and/or extensive
experience in a case management role in the
community care sector
/gAbility to undertake comprehensive assessments
and develop care plans
/gAbility to maintain accurate client records
/gHighly developed verbal and written
communication skills
/gExperience in managing and supervising support
staff
/gHighly developed relationship management skills
/gWell-developed knowledge of local networks
/gAbility to use electronic reporting tools
/gIntermediate skills in Word and Excel
/gAbility to work autonomously
/gWillingness to travel
/gCurrent full NSW drivers license

About the Role

Duties & Responsibilities
Develop and execute the organisation's Marketing and Communications Plan
Brand awareness and implementation
Coordinate the flow of multiple communications and publications
Create and maintain the organisation's newsletter, marketing materials,
brochures, and other publications as requested
Design, edit and produce flyers, forms, ads, templates and other material
Actively drive Facebook, Twitter and other social media profiles as
appropriate to grow the support base
Research and source up-to-date information on industry developments

Maintain the integrity of the database and extract necessary database

Essential criteria
Minimum 2 years recent experience in a similar position
Relevant tertiary qualification
Demonstrated experience in writing copy for marketing collateral including
newsletters, brochures, annual reports etc, and producing material from start
to finish.
Experience in publication management, including the preparation of content,
and the coordination of design, print and distribution
Demonstrated experience in dealing with the media, writing media releases
and feature articles
Demonstrated experience in website and database management
Experience writing, producing and delivering print and online
communications, social media and email marketing materials
Strong graphic design skills and ability to use a range of design software
programs to create our publications in-house
Intermediate knowledge of the Microsoft Office suite (including Word, Excel
and Outlook)
Exceptional written and verbal communication skills
Excellent organisational and time management skills and proven ability to
meet deadlines
Excellent proofreading skills
Ability to work in a team and independently
A desire to enhance the lives of our clients
Desirable criteria
Previous experience working within the not-for-profit sector
